Your insurance is your responsibility

Your insurance is your responsibility. To help us help you, please do the following:

Make sure to bring all of your insurance information , including your identification card. We cannot bill your insurance without this information.
Your insurance company requires you to obtain a referral to see one of our Doctors. It is your responsibility to call your insurance company to find out what their requirements are, and have your Primary Care Physician provide a valid referral authorization number.
Be prepared to pay your Co-Pay, as required by your insurance company, at the time of service.

A Note About Insurance

We are not a party to the contract between you and your insurance company — only your insurance company, or your employer’s Human Resources department, can answer questions regarding your benefits. Please be aware that some services provided may be “noncovered”, or may not be considered reasonable and necessary by your insurance company. We suggest that you review your policy in full so that you understand what services will be covered by insurance, and what you may be responsible for.

A Note About Referral Authorization

Do you have a valid authorization number from your primary care physician?

Most insurance companies require a valid referral at the time of service – that means at your very first appointment. It is your responsibility to obtain this before you come in.

If you do not have a valid authorization number, your insurance company will not pay for your visit. Referrals are not given retroactively. Because of this, we must comply with the following insurance requirements:

Your referral must be brought to your first visit.
Only you can obtain a referral from a primary care physician (PCP).
We cannot bill your insurance company if you do not have a valid referral at the time of your visit, and will have to reschedule your visit.
Verbal confirmation of a referral from your PCP’s office is not acceptable. We need a hard copy of the referral. Your PCP can fax the referral to our office at 303-321-8814.
Patients without a referral, or without adequate insurance information, may be seen on a fee-for-service basis. Payment must be made at the time of your visit.
Co-payments must be paid at the time of service.
